The ANA (antinuclear antibody) test is the most common screening test for autoimmune diseases like lupus. A positive ANA causes a lot of anxiety, but on its own it is not a diagnosis.

Why is ANA positive in healthy people?

Up to 15–20% of healthy people, especially women and older adults, have a low-positive ANA. It can also be positive after viral infections, with certain medicines, and in thyroid or liver disease.

Understanding your report

• Titre: 1:40 or 1:80 is often seen in healthy people. 1:160, 1:320 or higher is more significant.
• Pattern: homogeneous, speckled, nucleolar, centromere and others. The pattern hints at which specific antibody tests to order next.
• ANA by IFA is the preferred method. Some ELISA reports are less reliable.

When does a positive ANA matter?

ANA matters when it comes with symptoms such as:

• Joint pain with swelling
• Rash on the face or sun-exposed skin
• Mouth ulcers, hair fall
• Fingers turning white or blue in cold (Raynaud's)
• Unexplained fever, fatigue
• Protein or blood in urine, low blood counts
• Dry eyes and dry mouth
• Repeated miscarriages

Your rheumatologist may then order tests like anti-dsDNA, ENA profile, complement levels (C3, C4) and urine examination to reach an exact diagnosis.
