Systemic lupus erythematosus (SLE or lupus) is an autoimmune disease that affects women nine times more often than men, usually between 15 and 45 years. Lupus is called "the great imitator" because its symptoms resemble many other illnesses, which often delays the diagnosis.

Common symptoms

• Butterfly rash across the cheeks and nose, worse after sun exposure
• Joint pain and swelling in hands, wrists and knees
• Hair fall and patchy thinning
• Painless mouth ulcers
• Prolonged fever and extreme tiredness
• Fingers turning white or blue in the cold
• Swelling of feet or frothy urine (kidney involvement)
• Chest pain on breathing deeply
• Low blood counts; repeated miscarriages in some patients

Tests

ANA is positive in almost all patients. Diagnosis is confirmed with anti-dsDNA, complement (C3, C4), blood counts and urine tests, combined with the clinical picture.

Living well with lupus

• Hydroxychloroquine is the backbone of treatment for most patients and reduces flares; regular eye check-ups are advised while on it
• Other immune-modulating medicines are added depending on which organs are involved
• Sun protection: sunscreen, full sleeves, umbrella
• Regular urine and blood monitoring even when you feel well
• Plan pregnancy with your rheumatologist. Most women with controlled lupus can have healthy babies.
